Documentation and Reporting

  • Medical Records:

We provide a complete and organized set of medical records, which include all in-office evaluations, treatment notes, and progress reports. Additionally, we procure and incorporate all relevant diagnostic tests, hospital records, and specialist reports. This includes past medical history and primary care records, even if those providers are not actively treating the patient for the specific injury presented to our office.

  • FCE Reports:

Our Functional Capacity Evaluation reports include a complete medical examination, a thorough review of all medical documentation, and video footage of the evaluation process. Each FCE is conducted with clinical rigor, including the use of blood pressure monitoring, pulse oximetry, respiratory rate, and heart rate tracking. Patients wear a Polar heart rate monitor during all physical components of the testing to ensure real-time data collection and physiological tracking.

  • Timely Updates:

Patients undergo re-evaluations every 30 days to assess clinical outcomes, make treatment modifications, and update or review relevant diagnostic or specialist findings. These follow-ups are thoroughly documented in each report. When MRI studies yield positive findings, the images are included in the interim and final reports to provide visual confirmation of pathology and further support the medical opinion.
